| Those selected to play for the VP
Hockey team must meet the following requirements: |

|
Be enrolled as a student at Villa
Park High School, or independent studies/continuation
& live within the Orange Unified School District. |
|
|
Maintain a current GPA of at least
2.0, with no "U's" in citizenship in any
grading period. |
|
|
Agree to comply with the "VP
Hockey Player Code of Conduct". |
|
|
Sign Liability Release/Assumption
of Risk form, and have parents/guardians sign
as well (for players under 18). |
|
|
Complete the Emergency Information/Medical
Consent form. |
|
|
Pay
a fee of $400 along with a small uniform fee to
defray the cost of customized jerseys and uniform
pants. Checks should be payable to "VP Hockey". |
|
|
Be in good health with no medical
conditions or injuries that would prevent participation
vigorous athletics. |
